Cindy Krafft, PT, MS
Cindy has 17 years of home health experience ranging from PRN Clinician to the Director of Rehabilitation for a six agency home care system. She serves as the President of the Home Health Section of the American Physical Therapy Association and is immediate past Chair of the NAHC Therapy Advisory Committee. She has authored two books focused on Therapy Documentation and Interdisciplinary Care Management as well as a variety of articles in Caring Magazine, The Remington Report, Success in Home Care, Home Healthcare Nurse, and the Home Health Section of APTA newsletter. As well as being an expert on therapy practice in home care she also assists agencies with achieving OASIS competency. She served as the Clinical Co-Director of the Delta National OASIS-C Best Practices Project and the Clinical Director of the Delta Excellence in Therapy Project. She is a well-received speaker at both the state and national levels on the topics of OASIS, therapy documentation, program development, therapy utilization, and recruitment.

Rhonda Will, RN, COS-C, BCHH-C
Rhonda has experience as a registered nurse since 1971 and has worked in home health care since 1979 in various clinical, administrative and management roles. Rhonda has extensive experience as a trainer. Her areas of expertise include PPS and OASIS assessment skill building, documentation, care planning and care management, regulatory compliance, and policy and procedure development.

Cindy Campbell, RN, BSN, MHA-Clinical Informatics (c)
Cindy Campbell is a nationally recognized leader, speaker and management consultant to the full continuum of healthcare at home. Cindy leads a team of executives who conduct organizational design, operational improvement and clinical model advancement for Home Health, Hospice, Advanced Illness Management and Private Duty organizations throughout the United States. She holds a proven track record of improving clinical and financial outcomes while increasing employee engagement. Cindy directs Fazzi’s Operational Consulting and the Fazzi Total Management System, teaching executives and managers functional management methodology, targeted to the continuum of care at home. Cindy is a passionate advocate for advancing practice through innovation, increasing the integration of technology into operations and clinical practice. Cindy is a past Board Member of the National Association for Homecare & Hospice, chaired the Pediatric Homecare & Hospice Association of America and sat on the American Telemedicine Association’s Executive Committee-Homecare and Remote Monitoring Special Interest Group.

Nancy Buseth, RN, PT
Nancy is a Senior Consultant for Fazzi Associates, Inc. She has had 14 years of home health experience ranging from staff therapist to Director of Rehabilitation and Director of Referral Management and Community Relations. She was the team leader for a Service Excellence program for patient satisfaction and also a member of the Performance Improvement team. Her experience also includes OASIS education for both therapists and clinicians, along with therapy documentation training. She has done hundreds of record audits for OASIS compliance and accuracy and education for staff reviewing records. She has led projects for Fazzi Associates involving dozens of agencies for both Therapy documentation and OASIS compliance. She has published two articles for Caring Magazine and Home Health Line. She has been a speaker at both the National and State level for home care. She was a core member for a Care Navigation team within a hospital system to decrease re-hospitalizations. She worked as an RN in the field of Geriatrics prior to getting her Physical Therapy degree.

Claudia Baker, RN, BS, HCS-D
Claudia has spent 30+ years as RN with significant experience in various field and management roles in home health, hospice and private duty settings. She has significant experience in product development and support for clinical software related to each of these key community-based areas. Claudia has extensive on-site interaction and education at agency level, specifically related to workflow process and improvement. She provides web-based educational offerings to agency audiences, both large and small, paying specific attention to the individual needs of each learner and agency.

Overview of Homecare

Home care has a language all its own, and with that comes expectations that are unique to the industry. In this introductory session we will show participants how to navigate the basics of home care, from mastering the assessment to building stronger communication to ensuring proper reimbursement. *Approved for 1.25 contact hours

Home Care Rules and Regulations
1.25 Hours

Although not the favorite topic for most clinicians, knowing the rules and regulations that impact home care is essential to ensuring that clinical practice aligns with the expectations of payers such as Medicare and other regulating entities. In this session we'll cover the fundamental rules of home care, including the Medicare Conditions of Participation, HIPAA, and more. *Approved for 1.25 contact hours

Managing the Patient in the Home Setting
1.25 Hours

Learning to manage a home care patient outside the boundaries of the facility setting presents unique challenges and opportunities for collaboration. In this session we will help participants better understand the discipline specific practice issues that apply to the entire team and methods of collaboration designed to help build more effective care plans. *Approved for 1.25 contact hours

Defining and Documenting "Medical Necessity"

Documentation is a necessary component of patient care. The scrutiny on home care is high and all clinicians must be clear as to what is “skilled care” and how to document the indispensability of the services provided. In this session we will discuss Medical Necessity and outline the documentation requirements for supporting it. *Approved for 1.25 contact hours
